The staff of Hidden Harbors will conduct a Resident Assessment prior to admission. We do this so that we can ensure you will receive the most appropriate care. If we cannot accommodate you here, we will do our best to assist you in finding the appropriate care you need.
The Resident Assessment:
- Is a tool created by the State of Michigan used to assess a possible resident to ensure that the care is appropriate for that resident.
- This tool helps us to know the amount of personal care, supervision, and protection that is required by each resident is available in this facility.
- The Assessment reveals the kinds of services, skills, and physical accommodations that are required of Hidden Harbors to meet the resident's needs.
- We do our best to ensure that all our Residents appear to be compatible with other residents and members of the household.
All Residents are admitted because they have been evaluated by our Administrator and through that evaluation have been found to be capable of living in an Adult Foster Care Large Group Home setting.
All applicants need to:
- Submit a completed application including all required attachments, and any known medical information.
- Agree to a resident assessment prior to admission by the staff of Hidden Harbors to be authorized as admissible.
- This Assessment should take no longer than an hour.
- Have a proven funding source to pay for care.
All Residents who call Hidden Harbors home, must be able to:
- Eligible for the level of care provided by a large group home adult foster care facility as required by the State of Michigan Licensing and Regulatory Affairs and approved by a physician.
- At least 60 years of age or older; or has acquired a waiver by the State of Michigan Licensing and Regulatory Affairs to live in an Adult Foster Care setting under the age of 60.
- Hidden Harbors may be able to help you get a waiver, please ask the Administrator
Residents who require more assistance with the above, can be reevaluated and receive more care within Hidden Harbors at an additional cost, including, but not limited to memory care.
- Applicants are screened based on the level of care needed. The level of care should include a diagnosis from the physician stating that Adult Foster Care is a beneficiary placement. Other evaluations suggested by the applicant's physician, such as psychological or intelligence tests may also be used in screening.
- For all the Resident Applicants, there shall be no discrimination, in the decision process, regarding race, color, sex, religion, national origin, political affiliation, age, weight, size, or any other protected population.
- The applicant must be able to have their needs met by Hidden Harbors and must be able to participate in group home activities without limiting the delivery of services to other residents.
- The applicant must agree to abide by the rules and regulations of the facility.
